In order to comply with the requirements of British Triathlon and sporting legislation it is important that all juniors who are attending junior training complete the following form. Information on the form will remain confidential and access will be restricted to relevant coaches and members of the club’s committee. By signing this form you have disclosed below any medical conditions that may affect your child’s ability to take part in training sessions for SWIMMING, CYCLING and RUNNING or other strenuous activities hosted by the club and by submitting this form, you bear no responsibility to the club for incidents that may occur due to non-disclosure of a condition.
